Since the outbreak of Covid-19, we have temporarily changed how we operate appointments in the practice in order to minimise the risk of spreading the virus and therefore protecting patients and staff alike. Please read the following information.
Please be patient with our staff as there is a huge increase in workload, particularly with phone calls at this time.

Prevention and self-isolation

If possible, use our email service firstname.lastname@example.org or visit our website churchstreetmedicalcentre.ie where there is a facility to fill out our questionnaire in relation to Covid-19 symptoms and also a facility to email your prescription request. Please be aware that these will not be dealt with over the weekend.
We are making every effort to minimise the risk of spreading Covid-19 so we are now limiting appointments to patients with acute medical problems.
If you have medical problems that need urgent attention please contact the practice and we will arrange for you to be seen. We do not, however, encourage any routine reviews. These can be postponed until after the crisis.
If you suspect you have Covid-19 please fill out our online questionnaire on our website (https://churchstreetmedicalcentre.ie/). When this form is submitted, it will be reviewed by one of the doctors. We will then contact you by phone to address your problems.
When you arrive at the surgery for your appointment, please contact our secretary by phone. We will ask you to wait in the car until the doctor is ready. You will then be contacted by phone to come into the surgery.
The purpose of this change is to protect you from contracting Covid-19.
We would ask you to be patient and understanding at this time.
GP Video Consultations Available
In order to manage the Covid-19 outbreak we have introduced online GP consultations. These are particularly useful if you are suffering from possible Covid-19 symptoms. This is also valuable for managing long-term illnesses as an alternative to consultations in the surgery.
The process is simple. You can phone to arrange an appointment. You will then be sent a link by text or email. Just before the appointment time, you can click on the link and you will enter a virtual waiting room.
Your GP will then begin the video consultation. You need to have google chrome for android phones or safari for iOS phones to connect with our system.
Ensure you have a safe and quiet area where you can talk privately about your symptoms. Remember that the consultation is limited as we cannot examine you but is still a better alternative than a phone consultation.
Testing for Covid-19
If appropriate, and where there is clinical concern, your GP may refer you for the Covid-19 test. These are carried out in specialist testing centres. There have been significant delays in getting results.
Most routine blood investigations have been cancelled. We will continue to perform urgent blood tests and also certain necessary tests such as warfarin monitoring.
You can order your prescriptions in a dedicated area on our website (https://churchstreetmedicalcentre.ie/). Alternatively, you can make the request by email (email@example.com), letter or by dropping a request through the letter box in the surgery. We would ask that you do not request your prescription by phone as it is important to keep the lines open for urgent medical problems.
Please allow 48 hours for collection.
We will continue providing childhood immunisations as these are vitally important to protect your child.
Medical certificates/social welfare certificate
You can now make your application for Covid-19 related illness benefit payment online at https://services.mywelfare.ie/.
We will be completing all social welfare certificates online.
For illness benefit in cases of non Covid-19 related illness, you will need to forward the IB1 application form.

Where a medical certificate is required for your employer, we can supply a copy of the social welfare certificate.

Unfortunately, due to the hugely increased workload currently, there will be a delay in provision of these forms.

Please note that all Covid related consultations are free of charge.
There is a charge, however, of €20 for private prescriptions.
